1. Please fill in the online application form with clear and complete information in font of Simplified Chinese (half-width). Shortening and full-width font should be avoided.

3. The name filled in must be consistent with the one used on ID card or passport;

　　Pinyin forms of “Surname” and “First Name” should be in Chinese Pinyin or English, and initial letter be capitalized, eg.

4. Photograph should be recent-taken 1 inch normal chromophotograph without hat.
5. “Date of birth” should be in form like this: |01|07|1970|
6. “Dress size”: S 　 M 　 L 　 XL 　 XXL 　XXXL
7. Issuing date and Expiry Date of Certificate: |01|07|2000|
8. Graduation Date: July, 2009.
9. Make sure that the primary telephone number includes country/ 　　region code for contact convenience.

10. For the three items: “Within the past five years, have you been involved in any volunteer program”, “Have you ever worked or lived with a disabled person”, and “Within the past five years, have you been awarded by any government agency or social organization?”, if you have not, just skip them and fill in the corresponding blanks with “No”; if you have, please continue filling in with correlative information.
11. For “Have you got a medical license” and “Have you got a driving license”, if you have not, just skip them and fill in the corresponding blanks with “No”; if you have, please continue filling in with correlative information.
12. If options of “A”, “B”, “C”, “D” are offered, choose one to fill in.
13. If options of “Yes/No” are given, fill in with “Yes” or “No”.
14. For the two items “Have you ever worked or lived with a disabled person” and “Within the past five years, have you been awarded by any government agency or social organization”, if you have, choose only one to complete.
15. Please fill in the “Highest educational level” blank with the academic degree to be given.
16. For “Language skills”, fill in the “Level” with blank with “Ordinary”, “Good” or “Mastery”. For “Certificate”, if you have been awarded with several certificates, choose one to fill in.
17. “Contact language” refers to in which language you would like to be contacted by the BOCOG.
18. Please fill in “Sport Skills” item with the sports skill you are good at; fill in the “Level” item with “Ordinary”, “Good” or “Mastery”.
19. For “Please choose the areas in which you are willing to work”, each item is limited to one area. Altogether three areas can be chosen. “Volunteer” refers to those engaged in the personnel management and service for the Olympics and Paralympics.
20. “Test event”: refers to more than 40 international sport events held in places like Beijing before the 2008 Olympic Games, which serve as the test events of the Olympic Games.
21. Please be done with all of the items. Fill in with “No” for blanks.
